Sage Village

About

Sage Village provides up to 6 months of transitional residential services that focus on behavioral health and recovery. The program offers enhanced support in a non-medical residential facility for adults experiencing a combination of behavioral health difficulties, substance abuse challenges, and related criminal justice involvement. Services are provided in a supportive environment that seeks to reduce recidivism through the development of coping skills, a focus on psychiatric care, and the promotion of alcohol and drug recovery services.

Interdisciplinary care teams work with participants to create individualized treatment plans to support their progress with educational, employment, social, and family goals. The program models and encourages empowerment, personal accountability, the development of meaningful life roles, and to help promote a successful transition back to community living. Individuals will be referred from inpatient hospitals, emergency departments, jails, and various outside agencies.
