Sacramento Pathways to Success After Homelessness

PATHWAYS TO SUCCESS AFTER HOMELESSNESS (PATHWAYS) is a large program comprised of three parts: adult mental health services, children’s mental health services, and housing support services. It is a Full-Service Partnership (FSP) program that provides comprehensive psychosocial rehabilitation, case management services, medication services, and housing services for people of all ages with a severe mental illness who are experiencing homelessness or are at risk of homelessness. The mission of the program is to stably house individuals and families so that they can realize their recovery goals, while supporting them in the least restrictive way possible and helping them improve their quality of life.
